To change your Federal Withholding Allowance fill out the W-4 form.

To change your State Withholding Allowance fill out the IT-4 form.
Remember to include your school district name on the IT-4 if you live in a taxing district.

If you do not have a Timesheet, download one from this site, fill it out and mail it in.
Don't forget to have your supervisor sign it.
You may want to make a copy for your records and/or the customer.

If you would like to sign up for Direct Deposit, fill out the direct deposit form, attach a voided check and submit it to the payroll department. After it is received, direct deposit will become active in about two weeks.
